Pathologic complete response (pCR)
In one sentence
Pathologic complete response means no residual invasive cancer in the breast or sampled regional nodes after neoadjuvant treatment.
The intuition
Think of the surgical specimen as the inspection after treatment. A scan asks what a region looks like; the pathologist examines tissue for remaining invasive cancer. These are different kinds of evidence. Neither an apparently empty scan nor negative blood DNA can stand in for that inspection.
How it works
A common breast-trial definition is ypT0/Tis ypN0. The prefix y means assessment after treatment; p means pathology. T0/Tis permits no residual breast tumor or remaining ductal carcinoma in situ (DCIS), while N0 requires no residual invasive regional-node disease. Individual protocols can define pCR differently, so retain the actual definition.
DCIS is cancer confined within ducts. It can remain under this pCR definition because it is not invasive cancer. That does not make its margins or local management irrelevant. Conversely, invasive disease in a sampled node prevents pCR even if the breast has no residual invasive disease.
pCR is associated with favorable outcomes, particularly in TNBC. Association in patient groups does not make it a guarantee of cure, nor mean every intervention increasing pCR improves survival by the same amount. Treatment-effect surrogacy is a separate question. A report should distinguish breast response, nodal response and the combined endpoint.
Why it matters in cancer
It separates a tissue response endpoint from an imaging or blood-assay result, and keeps residual DCIS and nodal disease explicit.
Worked example
A specimen has DCIS in the breast and no invasive disease in breast or nodes. It can meet ypT0/Tis ypN0. If a node contains a residual invasive deposit, it does not. A negative ctDNA (circulating tumor DNA) test does not change either conclusion.
Common confusions
- pCR is a tissue endpoint; ctDNA clearance is a blood-assay finding.
- Residual DCIS may meet the common pCR endpoint but still affects local care.
- pCR is not a promise of cure; non-pCR is not a forecast of inevitable recurrence.
